Is Annie Wilkes (from the film Misery) intelligent?

I just watched Misery (creepy freaking movie) and something confused me. At times Annie (the antagonist) can seem to be slow witted and not overly bright, where at other times she can be highly perceptive, cunning, and capable of thinking quickly. She also has medical experience. Is she actually intelligent?
